Ayurvedic management of post rheumatoid arthritis knee joint contracture
Rational: Post rheumatoid arthritis knee joint flexion contracture is common complication and greater than 80° contracture challenging to manage. It is a common complication which occurs after a prolonged course of the rheumatoid arthritis. Svedana reduces sandhijadya (stiffness or contracture), gaurava (heaviness) and shaitya (coldness) as well as āma by its āmapācana properties.
Presenting concerns: A 45 years old female patient of āmavāta (rheumatoid arthritis) with knee joint contracture deformity was hospitalized in P. D. Patel Ayurveda Hospital, Nadiad for 2 months. She could not able to walk since 1 year due to contracture of both knee joints. She came on wheel chair and was unable to walk even with support. She was advised for the contracture repair surgery which she refused.
Intervention: She was treated with nāḍīsvedana 2 times a day for 20 minutes each. Simultaneously passive stretching 45 seconds in every 5 minutes interval was done. She was treated for 2 months. Patient was instructed to continue other Ayurvedic remedies given as the āmavāta (rheumatoid arthritis) treatment. After nāḍīsvedana goniometric assessment of the knees contracture was performed every week.
Outcomes: She got satisfactory result in stiffness and able to walk with support. Extension of both knee joints was improved up to 20° and increased range of motion. Her height was also increased up to 1.5 cm due to improvement in the extension of the knee joints with better feeling in daily activities.
